As a physician and now a disabled chronically ill person, I have had a great deal of contact with insurance companies. Much of it very bad. I do not believe that regulation is the answer. Insurance is a financial product with a goal of making money. It is too hard to make money in healthcare so they will always cheat. A single payer national system is the only answer. Yes, other countries have problems with their systems. Long waits, budget issues. The same problems we have. But when you are sick you get care and are not charged. People love their national health care in other countries. They do not hate it as we do here. And no, it is not socialism. We have government run army, infrastructure, police, fire fighters, science and many other things that need to happen and cannot depend on profit. Healthcare is no different.
